2.     Photostat  Copy of  Provisional  Certificate, Tamilnadu Medical Council Registration Certificate. For candidates with double initial an  affidavit in Rs.20/- non-judicial  stamp  paper from the Notary Public must be enclosed.

 

3.     Demand Draft for Rs. 300/- drawn in favour of The Registrar, The Tamilnadu Dr. M.G.R. Medical University payable at Chennai.

          

4. A self addressed sufficiently stamped envelope for receiving the certificate from the University.

 

 

 

RETOTALLING     

  1. Candidates of all Post Graduate and Under Graduate courses shall be eligible to apply for retotalling  of theory answer  paper  marks.

  2. Retotalling of theory answer paper marks is  permissible  only for those  candidates who have passed in clinical/practical examination and failed in theory paper.

  3.  Application for retotalling should reach the University within the prescribed date.

  4.  Application should be sent to the University  through the college.   Applications submitted directly to the University by the students shall be  rejected.

  5. A Demand Draft of Rs.500/- (Rupees  Five hundred only) drawn in favour of   The  Registrar, The Tamilnadu  Dr. M.G.R. Medical University                  69, Anna Salai, Guindy,Chennai - 600 032.   towards the retotalling  fee for  each subject be sent along with the applications

  6. The fee once paid will not be refunded at any cost. Any applications sent in this regard should   be sent  after careful scrutiny regarding the eligibility of the candidate

  7. A  Xerox copy of the Mark statement should be enclosed along with the application.

 4.      Xerox Copy of  SSLC,  HSC, Provisional Certificate, Mark Statement

         and Degree Certificate (if available).

Duplicate   Degree   Certificate

1.     Non traceable Certificate from the Police Station with round seal. (in which area the certificate lost by the candidate )

4.     All Xerox Copy of   SSLC,  HSC, Provisional Certificate, Marks Statements, Photo Identity card issued by the Institution / Employer and Degree Certificate, if available.

 

5.   a) Bonafide Certificate duly affixing latest photo with candidate's signature attested by the Principal of the Institution.

 

    b) Two copies of candidate’s latest passport size colour photograph put in a small envelope .(Name, Course, Register Number should be written on the back side of one photo copy) 

 

      (b) To put his/her full specimen signature in BLACK INK in the middle of the sticker label/white sheet (3 Signatures) in the size of 6" x 1.25" ( Do not write name or date with signature)

Verification of Educational Qualification/Degree Certificate

           For verification of educational qualifications/degree issued by this University, the Employer/Institution may send a written request with reason and copy of  the Provisional Certificate (or) Degree Certificate with Registration Number   assigned by the University to the following address.
